A fully developed claim is VA’s name for a claim you send with the records you already have. This is a short checklist. You still get those records to VA. We do not file.

Use records you already have. A video visit is still a private record if that is how you got care. VA’s page says private medical records. It does not use the word telehealth.

Personal and witness statements

A personal statement is your own words. A witness statement is from someone who saw what happened. You write them. We do not write them.

You can open the official statement forms below. You fill them out. We do not fill them in. We do not send them.

Forms you open yourself

The disability application is VA Form 21-526EZ, Application for Disability Compensation and Related Compensation Benefits. You open the form page or the fillable PDF. You fill it out and send it to VA.

Post-traumatic stress disorder (PTSD) also uses VA Form 21-0781. Open that form on the PTSD page. Traumatic brain injury (TBI) and migraine use VA Form 21-526EZ only. There is no form that is only for a brain injury.

You fill this out and send it to VA. We do not fill it in with your records. We do not send it. We do not make a packet that is ready to file.
